As I have a big database with ~40 000 entries listing for me relevant notes, I try to figure out how to import the different fields into MS.
So i struggle with the mapping of both database entries and also can't find how the CSV field names are named (and could renamed by me) in MS.
If custom up to five is possible, how can I find them in MS after Import?
Can I add new fields in MS and if so, how?

You can't currently add new fields - there's just the one custom field. I don't know where you saw custom3 through 5 - was that just talking about future changes? I do want to support more custom fields in the future, but there is just Custom, Custom2, and Custom Group at the moment.
