Add the same song to a setlist more than once.

Add a setlist-level note to the song.

Notice that the note appears for both instances of that song in that setlist. So, within the setlist, the setlist-level note actually behaves like a song-level note.

It would be nice if setlist-level notes would be specific to the particular item in the setlist.

That's not possible - the database is not structured that way. You'll have to create a copy of the song if you need different notes listed for each copy of it. The song versioning feature later this year will address this, as you could then have multiple versions.
